To submit an entrance form for a team, the following forms will need to be submitted. Each file is in PDF format and require Acrobat Reader to open. You can download Acrobat Reader for FREE from Adobe's Web site. 1)2012 Bowling League Rules 2) Team Entrance Form due by 4/5/2012 3) Player Registration Forms for each player on the team due by 4/5/2012 4) Team Roster Form for each team due by 4/5/2012 5) Coaches Registration Forms must be returned with the Entrance Fee by 4/5/2012) 6) Player Eligibilty: Players must be in High School - Grades 9-12 Registration Requirements: $220 per team Please make check payable to: CYM Bowling.
